The Oak Street Clinic Telemedicine Portal allows you to use our secure, state-of-the-art, HIPAA compliant video conferencing software to be treated by one of our Board Certified Nurse Practitioners from the comfort of your home.

Telemedicine is a safe and cost effective alternative to the more traditional face-to-face way of providing medical care. We can treat many acute and chronic conditions remotely including:

A Telemedicine visit at Oak Street Clinic only costs $49. For many patients this is a cost savings, especially when you factor in the cost of a regular clinic visit which can include copays, deductibles, co-insurance, time out of work, time spent in the waiting room and travel costs.

It is with great sadness and heavy heart that we are writing to let you know that we will be closing the clinic at the end of September. It has been increasingly difficult to manage a small independent clinic in today’s current healthcare environment and have made the difficult decision to close permanently effective September 28, 2016.

We are recommending those in need of a new primary care provider contact the customer service phone number on the back of your health insurance cards for assistance in finding providers accepting new patients. Once you have found a new provider we will gladly forward a copy of your medical records upon receipt of a HIPAA compliant signed authorization to your new physician/provider to help facilitate the easiest transfer of care for you.

Additionally, if you would like a physical copy of your medical records we can provide these on a CD to you for $25 once we receive a signed authorization request. Online access to the patient portal also remains a great option and allows you to send your records directly to a new provider electronically. If you would like patient portal access you can request we provide this to you by calling us at 505.508.0197 or emailing us at info@oakstreetclinic.com to verify we have the correct email address you’d like to use for activation. The process for this is very quick and user friendly.
